SURRENDER OF INDIAN CITIZENSHIP
General Information
Surrender Certificate:
The Indian Citizenship Act does not allow dual citizenship. As per the Passport Act, it is mandatory for all Indian passport holders to surrender their last Indian passport to the Consulate immediately upon acquiring foreign nationality.
Holding an Indian passport after acquiring foreign nationality is a legal offense under both the Citizenship Act and the Passports Act.
Once the Indian passport is surrendered, the Consulate will cancel and return the passport along with a Surrender Certificate.
Please note: No consular services such as OCI, Visa, etc. can be provided unless the Surrender Certificate (or a Declaration Certificate in applicable cases) is submitted.
Declaration Certificate:
This certificate is issued to individuals who have lost or misplaced their last Indian passport and are therefore unable to physically surrender it.
In such cases, a Declaration Certificate is issued upon submission of a notarized sworn affidavit regarding the loss, along with supporting documents.
Important: Surrender Certificate or Declaration Certificate is mandatory for submitting an OCI or Visa application.
How to Apply for a Surrender Certificate
A. Steps to Apply:
a. Fill out the Application Form

Visit: https://portal5.passportindia.gov.in/Online/index.html

Step 1: Register by selecting "Indian Mission Peru – Lima" and create a User ID and Password.

Step 2: Log in using your credentials, select the Surrender Certificate application form, complete it online, print the form, affix a passport-sized photo, and sign it.
b. Submit the Physical Application
Submit the printed and signed application form along with the required documents in person at the Embassy.
